Intimation of receipt of foreign contribution by way of gift / as Articles / Securities / by candidate for Election (FC -1)
This form is used to file for reporting the following information if it exceeds 1 lakh.
• Intimation of receipt of foreign contribution by way of gift from relative by an individual
• Intimation about Foreign Contribution by way of (Articles) from relative by an individual
• Intimation about Foreign Contribution by way of (Securities)
• Intimation to the Central Government of Receipt of Foreign Contribution received by a candidate for Election
• Intimation about Foreign Contribution by way of (Articles) from relative by an individual
• Intimation about Foreign Contribution by way of (Securities)
• Intimation to the Central Government of Receipt of Foreign Contribution received by a candidate for Election
Form FC-3A
Application for FCRA Registration (FC-3A)
This service is applicable for ‘prior permission’ under sub-section (2) of section 11 of the Foreign
Contribution (Regulation) Act, 2010 (42 of 2010) for the acceptance of foreign contribution by a
person/association having definite cultural, economic, educational, religious or social programme:
Document to be submitted
• Registration Certificate of Association(self certified by chief functionary).
• Memorandum of Association/Trust Deed(self certified by chief functionary).
• Activity Report for the Last 3 Years.
• Audited Statement of accounts for the last three years.
• Memorandum of Association/Trust Deed(self certified by chief functionary).
• Activity Report for the Last 3 Years.
• Audited Statement of accounts for the last three years.
Form FC-3B
Application for FCRA Prior Permission (FC-3B)
This service is applicable for ‘prior permission’ under sub-section (2) of section 11 of the Foreign Contribution (Regulation) Act, 2010 (42 of 2010) for the acceptance of foreign contribution by a person/association having definite cultural, economic, educational, religious or social programme:

• Registration Certificate of Association(self certified by chief functionary).
• Memorandum of Association/Trust Deed(self certified by chief functionary).
• Commitment Letter from the donor organization and agreement.
• Project Report for which FC will be received.
• Memorandum of Association/Trust Deed(self certified by chief functionary).
• Commitment Letter from the donor organization and agreement.
• Project Report for which FC will be received.
Form FC-3C
Application for Renewal of FCRA Registration(FC-3C)
This service is applicable for application for ‘renewal’ under section 16 of the Foreign Contribution (Regulation) Act, 2010 (42 of 2010) for the acceptance of foreign contribution by an person/association having definite cultural, economic, educational, religious or social programme:

• Registration Certificate of Association(self certified by chief functionary).
• Memorandum of Association/Trust Deed(self certified by chief functionary).
• FCRA Registration Certificate of association issued by MHA.
• Memorandum of Association/Trust Deed(self certified by chief functionary).
• FCRA Registration Certificate of association issued by MHA.
Form FC-4
Intimation - Annual Returns (FC-4)
This service is applicable to all Trust, NGO, Society and Section 8 Companies who have taken FCRA registration required to file the return on annual basis:
Document to be submitted
• Duly signed and seal Charted Accountant Certificate (with C.A registration number).
• Declaration Certificate of Chief Functionary.
• Audited Statement of Accounts (It should contain Payment Account, Income and Expenditure Statement, and Balance Sheet)..
• Statement of Account from Bank duly certified by officer of such bank.
• Declaration Certificate of Chief Functionary.
